Chlorthalidone falls under ANTIHYPERTENSIVE DRUGS — the group Nicotinamide Riboside interacts with.

Theoretically, nicotinamide riboside may have additive effects with antihypertensive drugs, potentially increasing the risk for hypotension.
One small clinical study suggests that nicotinamide riboside can modestly reduce blood pressure in hypertensive patients. However, other clinical research did not find a reduction in blood pressure when compared with control.

Here's the basic idea: chlorthalidone already works to lower your blood pressure by helping your body shed extra salt and water. Nicotinamide riboside might, in theory, nudge blood pressure down a little too. Put them together and you could in principle see a slightly bigger drop. If that happens, you might feel lightheaded, dizzy, or a bit woozy when you stand up quickly.

I want to be honest about how thin this evidence is. It's theoretical, and the human studies are mixed. One small trial showed a modest drop in blood pressure, while others showed no change at all. For most people this is a minor concern. Still, if you take both, it's worth checking your blood pressure now and then and mentioning it to your pharmacist or doctor.

What Is Chlorthalidone Used For?

Brand names: Hygroton, Thalitone

Chlorthalidone is used to treat high blood pressure (hypertension) in adults. Lowering blood pressure reduces the risk of serious cardiovascular events, especially strokes and heart attacks. It can be used alone or combined with other blood pressure medicines.

Chlorthalidone is also used as an add-on treatment for edema (fluid buildup) caused by heart failure, liver cirrhosis, and kidney conditions including nephrotic syndrome, acute glomerulonephritis, and chronic renal failure. Thalitone is additionally indicated for edema associated with heart failure, liver cirrhosis, and renal disease including nephrotic syndrome.
