Metolazone falls under ANTIHYPERTENSIVE DRUGS — the group Nicotinamide Riboside interacts with.

Theoretically, nicotinamide riboside may have additive effects with antihypertensive drugs, potentially increasing the risk for hypotension.
One small clinical study suggests that nicotinamide riboside can modestly reduce blood pressure in hypertensive patients. However, other clinical research did not find a reduction in blood pressure when compared with control.

Here's the gist: metolazone is a water pill that lowers blood pressure by helping your body shed extra salt and fluid. Nicotinamide riboside (a form of vitamin B3) might nudge blood pressure down a little on its own. Put the two together and, in theory, they could stack up and drop your pressure more than expected.

I want to be honest about how soft this is. It's theoretical, based on how things work rather than strong proof, and even the human studies disagree. If anything happened, you'd notice things like feeling dizzy, lightheaded, or woozy when you stand up. If you'd like to try NR, just check with your pharmacist or doctor first, and watch how you feel early on.

What Is Metolazone Used For?

Brand names: Diulo, Zaroxolyn, Mykrox

Metolazone tablets are used to treat edema (excess fluid buildup) linked to congestive heart failure or kidney diseases — including nephrotic syndrome and conditions with reduced kidney function. They are also used to treat high blood pressure (hypertension), either alone or alongside other blood pressure medications.

Metolazone tablets may also be used during pregnancy when edema is caused by a medical condition rather than normal pregnancy changes — but routine use in otherwise healthy pregnant women is not appropriate.
