Triamterene falls under ANTIHYPERTENSIVE DRUGS — the group Nicotinamide Riboside interacts with.

Theoretically, nicotinamide riboside may have additive effects with antihypertensive drugs, potentially increasing the risk for hypotension.
One small clinical study suggests that nicotinamide riboside can modestly reduce blood pressure in hypertensive patients. However, other clinical research did not find a reduction in blood pressure when compared with control.

Here's the gist for combining nicotinamide riboside (NR) with triamterene, your potassium-sparing water pill. NR isn't broken down by triamterene or anything like that. The concern is simpler and very theoretical: some early research hints that NR might nudge blood pressure down a little, so stacking it on top of a medication that already removes fluid and can lower blood pressure could, in theory, add up.

In real life, this is a minor worry. The evidence is weak, and studies don't even agree that NR lowers blood pressure. If it ever mattered, you might feel lightheaded, dizzy when standing, or unusually tired. Just check in with your pharmacist or doctor before starting NR, and let them know if you notice those symptoms.

What Is Triamterene Used For?

Brand name: Dyrenium

Triamterene capsules are used to treat edema — excess fluid buildup in the body — linked to several conditions, including congestive heart failure, cirrhosis of the liver, and nephrotic syndrome (a kidney disorder causing protein loss and swelling). It is also used for steroid-induced edema, idiopathic edema (fluid retention with no clear cause), and edema due to secondary hyperaldosteronism (overproduction of a hormone that causes the body to hold onto sodium and water).

It can be used on its own or alongside other diuretics, either to boost fluid removal or to help preserve potassium levels. Both Dyrenium and Triamterene capsules share the same approved uses as described above.
