Acetaminophen, Codeine, Methocarbamol falls under CYTOCHROME P450 2E1 (CYP2E1) SUBSTRATES — the group Onion interacts with.
Animal research shows that taking onion powder inhibits CYP2E1. However, this interaction has not been reported in humans.

This combo product pairs acetaminophen (Tylenol) with codeine and the muscle relaxant methocarbamol. The piece of it that touches the onion question is acetaminophen, which the body partly handles through an enzyme called CYP2E1. In animal studies, concentrated onion powder seemed to slow that enzyme down a bit. In theory, slowing it could shift how acetaminophen is processed, but here is the reassuring part: this has never actually been shown in people.
Honestly, eating onion in normal food amounts is not something I'd worry about with this medicine. Where I'd pause is with high-dose onion supplements taken regularly. If you're considering those, just run it by your pharmacist first. And no matter what, keep your total acetaminophen within the daily limit to protect your liver.
