Acetaminophen, Codeine, Methocarbamol falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Chanca Piedra interacts with.

Theoretically, chanca piedra might reduce the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that chanca piedra extract increases CYP1A2 activity. Theoretically, chanca piedra might increase metabolism of CYP1A2 substrates and lower serum concentrations. This interaction has not been reported in humans.

This combination product pairs acetaminophen and methocarbamol (a muscle relaxer) with codeine for pain. Here's the honest picture: chanca piedra has only been shown in lab dish studies to speed up a liver enzyme called CYP1A2. That enzyme plays a minor role with these ingredients, so any real-world effect is very uncertain. This has never actually been reported in people.

If anything, a faster enzyme could lower drug levels slightly, which might mean a bit less pain relief. Codeine is a prodrug that mainly relies on a different enzyme (CYP2D6) to become morphine, so its punch isn't really driven by CYP1A2. I wouldn't lose sleep over this, but since codeine can cause drowsiness and constipation, just check with your pharmacist before combining them.
