Warfarin falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Panax Notoginseng interacts with.

Theoretically, taking Panax notoginseng might reduce the levels and clinical effects of CYP1A2 substrates.
Animal research shows that administering Panax notoginseng intravenously for 7 days before intraperitoneal injection of caffeine can decrease maximal blood levels of caffeine by 37%. This interaction was attributed to the ability of Panax notoginseng to increase the activity of CYP1A2.

Here's the tricky part with warfarin: it's a blood thinner with a very narrow safety window, so even small shifts in its level can matter. The lab and animal data we have suggest Panax notoginseng may speed up an enzyme called CYP1A2, which could lower how much warfarin stays in your body and slightly weaken its clot-preventing effect.

But there's a flip side. This herb is also traditionally used to affect bleeding and clotting on its own, so the real-world result is genuinely hard to predict. Your INR (the blood test that guides your dose) could drift either direction. This is mostly theoretical, based on early evidence, but with warfarin I'd play it safe. Please check with your doctor or pharmacist before combining them so they can watch your INR closely.

Theoretically, taking Panax notoginseng concomitantly with warfarin may increase the risk of bleeding.
Animal research shows that taking Panax notoginseng concomitantly with warfarin increases plasma warfarin levels, prothrombin time, and international normalized ratio when compared with control. In vitro research also suggests that Panax notoginseng may downregulate expression of cytochrome P450 3A4 enzymes, which may affect warfarin metabolism.

Here's the thing with warfarin: it's a finicky blood thinner where even small shifts can tip you toward too much or too little effect. The concern with Panax notoginseng is twofold. First, the herb itself may affect how your blood clots, so stacking it on top of warfarin could push you toward extra bleeding. Second, lab work hints it may slow down a liver enzyme (CYP3A4) that helps clear warfarin, which could let warfarin build up and raise your INR.

I want to be honest about the strength of this: it's mostly animal and test-tube data, not human studies, so it's more of a theoretical caution. Still, with warfarin I always play it safe. Watch for easy bruising, nosebleeds, or blood in your urine or stool, and please talk with your pharmacist or doctor before adding this herb rather than starting or stopping anything on your own.

What Is Warfarin Used For?

Brand name: Warfarin

Warfarin sodium tablets and Jantoven tablets are both used to prevent and treat venous thrombosis (blood clots in the veins) and pulmonary embolism (a clot in the lungs). They are also used to prevent clot-related complications in people with atrial fibrillation (an irregular heart rhythm) or who have had a heart valve replaced.

In people who have had a heart attack, warfarin is used to reduce the risk of death, another heart attack, stroke, or other clotting events. It does not dissolve an existing clot, but it helps prevent the clot from growing and stops new dangerous clots from forming.
