Fludrocortisone falls under IMMUNOSUPPRESSANTS — the group Picrorhiza interacts with.

Picrorhiza seems to have immunostimulating activity. Theoretically, picrorhiza may interfere with immunosuppressant therapy. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), and other corticosteroids (glucocorticoids).

Here's the idea behind this pairing. Picrorhiza appears to give the immune system a little nudge to be more active. Fludrocortisone (Florinef) is a corticosteroid, and one of the things steroids do is calm an overactive immune response. So in theory, an immune-boosting herb could work against what your medication is trying to do.

I want to be straight with you, though: this caution rests on lab and animal data only, not real patient studies, so it's more of a maybe than a proven problem. If you take Florinef for adrenal insufficiency or to hold up your blood pressure, you don't want anything quietly chipping away at its effect. Please check with your pharmacist or doctor before adding picrorhiza rather than changing anything on your own.
