Clomipramine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Polypodium Leucotomos interacts with.

Theoretically, Polypodium leucotomos might increase levels of drugs metabolized by CYP3A4.
In vitro and animal research suggests that Polypodium leucotomos can decrease the metabolism of midazolam by a mechanism possibly related to the inhibition of CYP3A4. However, a small study in humans suggests that taking Polypodium leucotomos with midazolam does not affect the metabolism of midazolam.

Here's the gist: your body uses a liver enzyme called CYP3A4 to help break down clomipramine. On paper, Polypodium leucotomos might slow that enzyme down a little, which could let a bit more clomipramine build up. Since clomipramine is an active drug (not a prodrug), more of it in your system could mean stronger effects like dry mouth, constipation, drowsiness, dizziness, or a faster heartbeat.

The good news is this concern is mostly theoretical. The lab and animal hints didn't hold up when researchers actually tested the fern in people. So a real-world problem is considered unlikely. Still, since clomipramine can be sensitive to changes, it's worth a quick chat with your pharmacist or doctor before you add this supplement.

Clomipramine — sold as Anafranil and under the generic name Clomipramine Hydrochloride — is approved to treat obsessions and compulsions in people with Obsessive-Compulsive Disorder (OCD). This includes adults and children/adolescents ages 10 to 17.

To qualify for treatment, the obsessions or compulsions must cause significant distress, take up a lot of time, or noticeably interfere with work, school, or social life.
