Dronabinol falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Pomegranate interacts with.

Theoretically, pomegranate might increase levels of drugs metabolized by CYP2C9.
Some animal and in vitro research shows that pomegranate juice inhibits intestinal, but not hepatic, CYP2C9 isoenzyme activity. However, clinical research shows that neither pomegranate juice nor pomegranate extract have a significant effect on CYP2C9 activity in humans.

Here's the gist: your body uses an enzyme called CYP2C9 to help break down dronabinol. Pomegranate has been looked at as a possible blocker of that enzyme, and in theory blocking it could let a bit more dronabinol build up. With this medication, higher levels might mean more drowsiness, dizziness, a feeling of being 'high,' or a faster heartbeat.

The reassuring part is that this is mostly a paper concern. Lab and animal studies hinted at it, but actual human studies showed pomegranate juice and extract did not meaningfully change CYP2C9 activity. So enjoying pomegranate as a food or juice is very unlikely to cause problems. If you're using concentrated pomegranate extract supplements, it's worth a quick word with your pharmacist or doctor.

Dronabinol also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, pomegranate might increase levels of drugs metabolized by CYP3A4, but most research suggests this interaction is unlikely to be clinically significant.
Pomegranate contains several polyphenols that have individually been shown to inhibit CYP3A4. However, there is contradictory evidence about the effect of whole pomegranate juice on CYP3A4 activity. In vitro, pomegranate juice significantly inhibits the CYP3A4 enzyme, with comparable inhibition to grapefruit juice. In an animal model, pomegranate juice inhibits CYP3A4 metabolism of carbamazepine and increases levels of carbamazepine by 1.5 times; however, in human volunteers, drinking a single glass of pomegranate juice 240 mL or taking 200 mL daily for 2 weeks does not significantly affect levels of the CYP3A4 substrate midazolam after oral or intravenous administration. Another study in healthy volunteers shows that consuming pomegranate juice 300 mL three times daily for three days also does not significantly affect levels of simvastatin, a CYP3A4 substrate This suggests that pomegranate is unlikely to significantly affect levels of CYP3A4 substrates in humans.

Here's the gist with pomegranate and dronabinol. Your liver uses an enzyme called CYP3A4 to break dronabinol down, and pomegranate has plant compounds that can slow that enzyme down in a test tube. In theory, slowing it would let dronabinol build up a bit, which could mean more of its usual effects like drowsiness, dizziness, a foggy or 'high' feeling, dry mouth, or a racing heart.

But here's the reassuring part: in actual human studies, pomegranate juice did not meaningfully change levels of other CYP3A4 drugs. So a normal glass of juice or eating the fruit is very unlikely to cause a problem. If you're using a concentrated pomegranate extract, just mention it to your pharmacist or doctor to be safe.

What Is Dronabinol Used For?

Brand names: Marinol, Syndros

Dronabinol — available as capsules (Marinol, Dronabinol) and as an oral solution (Syndros, Dronabinol oral solution) — is approved for two conditions in adults. First, it treats anorexia (severely reduced appetite) associated with weight loss in patients with AIDS. Second, it controls nausea and vomiting caused by cancer chemotherapy in patients who have not gotten enough relief from standard anti-nausea treatments.

Both uses apply to all available formulations, but dosing timing and meal instructions differ between the capsule and oral solution products. Always follow the specific instructions for the product you have been prescribed.
