Here's the simple version: pomegranate, especially as juice or concentrated extract, can nudge your blood pressure down a little on its own. Your combo pill already does that job, with hydrochlorothiazide (a water pill) and metoprolol (a beta blocker) working to lower blood pressure and slow your heart rate. Stack pomegranate on top, and the two could add up.

If your pressure drops too low, you might feel dizzy, lightheaded, tired, or a bit faint when you stand up quickly. This is a possible, moderate concern, and the evidence is fairly modest. A glass of juice now and then is usually fine for most people, but daily large amounts or concentrated supplements are worth running by your pharmacist or doctor first.

Theoretically, pomegranate might increase levels of drugs metabolized by CYP2D6.
In vitro, pomegranate juice inhibits CYP2D6. However, the clinical significance of this potential interaction in humans is not known.

Here's the gist for your combo pill: the metoprolol half is cleared by a liver enzyme called CYP2D6. In a lab dish, pomegranate juice slowed that enzyme down. If that happened in your body, metoprolol could build up a bit and push its effects stronger, meaning your heart rate or blood pressure might drop more than expected. That could show up as feeling tired, dizzy, lightheaded, or unusually slow-pulsed. The hydrochlorothiazide (water pill) portion isn't really affected by this.

The honest truth is this is only test-tube evidence, and we don't know if it matters in real people. Enjoying pomegranate or its juice as food is very likely fine. If you're thinking about concentrated extracts or large daily amounts, just check with your pharmacist or doctor first.
