Here's the simple version: pomegranate, especially as juice or concentrated extract, can nudge your blood pressure down a little on its own. Since losartan (Cozaar) is already working to lower your blood pressure, adding pomegranate could add to that effect. It's not a dangerous combination for most folks, but the two together might lower your numbers more than expected.

If your pressure dips too low, you may feel dizzy, lightheaded, or a bit tired, especially when standing up quickly. The evidence here is modest and mostly theoretical, and an occasional serving of the fruit is unlikely to cause trouble. If you drink pomegranate juice daily or take an extract, mention it to your pharmacist or doctor so they can keep an eye on your readings.

Some animal and in vitro research shows that pomegranate juice inhibits intestinal, but not hepatic, CYP2C9 isoenzyme activity. However, clinical research shows that neither pomegranate juice nor pomegranate extract have a significant effect on CYP2C9 activity in humans.

Here's something interesting about losartan: it's actually a prodrug, meaning your liver uses an enzyme called CYP2C9 to switch it into its more active form that does most of the blood-pressure-lowering work. The theory is that pomegranate might slow that same enzyme down. If that happened, you could end up with slightly less of the active form, which in theory might make losartan a touch less effective at controlling your blood pressure.

The good news is that this is mostly a theoretical concern. Lab and animal studies hinted at it, but human studies showed pomegranate didn't meaningfully change CYP2C9 activity. Enjoying pomegranate as a food or normal juice is very unlikely to cause a problem. If you're thinking about concentrated extracts, just check with your pharmacist or doctor first.

Theoretically, pomegranate might increase levels of drugs metabolized by CYP3A4, but most research suggests this interaction is unlikely to be clinically significant.
Pomegranate contains several polyphenols that have individually been shown to inhibit CYP3A4. However, there is contradictory evidence about the effect of whole pomegranate juice on CYP3A4 activity. In vitro, pomegranate juice significantly inhibits the CYP3A4 enzyme, with comparable inhibition to grapefruit juice. In an animal model, pomegranate juice inhibits CYP3A4 metabolism of carbamazepine and increases levels of carbamazepine by 1.5 times; however, in human volunteers, drinking a single glass of pomegranate juice 240 mL or taking 200 mL daily for 2 weeks does not significantly affect levels of the CYP3A4 substrate midazolam after oral or intravenous administration. Another study in healthy volunteers shows that consuming pomegranate juice 300 mL three times daily for three days also does not significantly affect levels of simvastatin, a CYP3A4 substrate This suggests that pomegranate is unlikely to significantly affect levels of CYP3A4 substrates in humans.

Here's the good news first: pomegranate and losartan (Cozaar) are very unlikely to cause any real problem together. The concern on paper is that some plant compounds in pomegranate can slow down CYP3A4, one of the liver enzymes that helps process losartan. In a test tube that looks a bit like grapefruit, but in actual human studies pomegranate juice didn't meaningfully change levels of other CYP3A4 medications.

One quirky detail about losartan: it's a prodrug, meaning your body converts it into its more active form. So if anything slowed that step, you might get a touch less blood pressure lowering, not more. Either way, the effect is expected to be tiny. Enjoying pomegranate as a food or juice is fine, but check with your pharmacist before using concentrated extracts.

What Is Losartan Used For?

Brand name: Cozaar

Losartan tablets — available as branded Cozaar and several generic products — are used to treat high blood pressure (hypertension) in adults and children 6 years and older. Lowering blood pressure reduces the risk of strokes and heart attacks. Losartan tablets are also used to reduce the risk of stroke in people with high blood pressure who also have left ventricular hypertrophy (an enlarged left side of the heart). Note: evidence suggests this stroke-risk benefit may not apply to Black patients.

Losartan tablets are additionally approved to protect the kidneys in people with type 2 diabetes, high blood pressure, and signs of kidney damage (diabetic nephropathy). Arbli, the oral suspension form, shares these same approved uses.
