Pomegranate + Mannitol
Mannitol brand names: Osmitrol, Bronchitol.
Pomegranate has a moderate interaction with Mannitol. Theoretically, taking pomegranate with antihypertensive drugs might increase the risk of hypotension.

Here is the interaction we found between these two, with the evidence behind it.
ANTIHYPERTENSIVE DRUGS
Why this applies to Mannitol
Mannitol falls under ANTIHYPERTENSIVE DRUGS — the group Pomegranate interacts with.
Consuming pomegranate juice can modestly lower blood pressure.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture with pomegranate and mannitol. The interaction on file is about blood pressure. Pomegranate juice may gently lower it, and since mannitol pulls extra fluid out of your body as urine, it can lower blood pressure too. Stacking the two could nudge your pressure down a bit more than expected.
In real life, mannitol is usually given in a hospital or clinic setting (for things like brain swelling or certain eye and kidney situations), so your team is already watching your fluids and pressure closely. The evidence here is on the weaker, theoretical side, and food amounts of pomegranate are unlikely to be a big deal. Still, if you feel dizzy, lightheaded, or unusually weak, mention it. Always check with your pharmacist or doctor before adding concentrated pomegranate extracts.

From our pharmacist’s overview on the Mannitol page
“American hellebore tops my worry list for anyone on mannitol (Osmitrol, Bronchitol), an osmotic diuretic used to draw fluid off the brain or, when inhaled, to clear mucus in cystic fibrosis. The plant's alkaloids drop blood pressure and slow the heart, a dangerous match for a drug that already shifts fluid and pressure quickly.”

About Pomegranate
What is Pomegranate?
- Heart and blood vessel health
- Antioxidant support
- High blood pressure
- Inflammation
- Digestive upset
- Skin health
Pomegranate is a nutrient-rich fruit that is high in antioxidants and is widely enjoyed as food and juice. Early research suggests it may support heart health and blood pressure, but the evidence is still limited and not strong enough to call it a proven treatment. As a food it is generally safe, but talk to your pharmacist or doctor before using concentrated extracts, especially if you are pregnant, breastfeeding, or take medications.

Mannitol Is Linked to Lower Levels of 7 Nutrients
Natural Medicines records Mannitol as a medicine that can lower these. This is not an interaction with Pomegranate — it is a separate effect of the medicine, and it is a reason some people are advised to supplement rather than to avoid one:
