Byvalson combines two blood pressure medicines: nebivolol (a beta-blocker) and valsartan (an ARB). Pomegranate, especially as juice, has its own mild blood-pressure-lowering effect. So the concern here isn't about how your body breaks the drug down. It's that two blood-pressure-lowering effects can stack on top of each other.
If your pressure drops a little too far, you might feel dizzy, lightheaded, tired, or faint, especially when standing up quickly. The evidence is fairly modest, and a normal serving of pomegranate as food usually isn't a big deal. Larger amounts of juice or concentrated extracts are more likely to matter. If you enjoy pomegranate regularly, just mention it to your pharmacist or doctor so they can keep an eye on your readings.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

In vitro, pomegranate juice inhibits CYP2D6. However, the clinical significance of this potential interaction in humans is not known.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Byvalson, which combines nebivolol (a beta-blocker) and valsartan. The nebivolol part is broken down by a liver enzyme called CYP2D6. In a lab dish, pomegranate juice slowed that enzyme down. If that happened in your body, nebivolol could build up a bit more than usual, which in theory might lower your heart rate and blood pressure more than expected. You might notice feeling dizzy, lightheaded, unusually tired, or sluggish.
But here's the honest part: this is only test-tube evidence, and we don't know if it actually matters in real people. Enjoying pomegranate as food or normal juice is very unlikely to cause a problem. If you're thinking about concentrated pomegranate extracts, just run it by your pharmacist or doctor first.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES
3 of 3Nebivolol Hydrochloride, Valsartan also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.
Some animal and in vitro research shows that pomegranate juice inhibits intestinal, but not hepatic, CYP2C9 isoenzyme activity. However, clinical research shows that neither pomegranate juice nor pomegranate extract have a significant effect on CYP2C9 activity in humans.

Here's the short version: this concern is mostly theoretical. Pomegranate can block a liver enzyme called CYP2C9 in lab and animal studies, but careful human studies show it doesn't really change how that enzyme works in people. Byvalson combines nebivolol (a beta-blocker) with valsartan (a blood pressure drug), and neither is a prodrug, so there's no flipping to worry about here.
What this means for you: if pomegranate did raise these drug levels even slightly, you might notice a bit more of their usual effects, like lower blood pressure, dizziness, or a slower heartbeat. In real life, enjoying pomegranate as food or juice is very unlikely to cause a problem. If you're using concentrated extracts, just mention it to your pharmacist or doctor.
