CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Selinexor
Selinexor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Pomegranate interacts with.
Pomegranate contains several polyphenols that have individually been shown to inhibit CYP3A4. However, there is contradictory evidence about the effect of whole pomegranate juice on CYP3A4 activity. In vitro, pomegranate juice significantly inhibits the CYP3A4 enzyme, with comparable inhibition to grapefruit juice. In an animal model, pomegranate juice inhibits CYP3A4 metabolism of carbamazepine and increases levels of carbamazepine by 1.5 times; however, in human volunteers, drinking a single glass of pomegranate juice 240 mL or taking 200 mL daily for 2 weeks does not significantly affect levels of the CYP3A4 substrate midazolam after oral or intravenous administration. Another study in healthy volunteers shows that consuming pomegranate juice 300 mL three times daily for three days also does not significantly affect levels of simvastatin, a CYP3A4 substrate This suggests that pomegranate is unlikely to significantly affect levels of CYP3A4 substrates in humans.

Here's the gist: Selinexor is partly broken down by a liver enzyme called CYP3A4. Some compounds in pomegranate can slow that enzyme in the lab, and the worry is that this might let Selinexor build up. Since Selinexor is an active drug (not a prodrug), higher levels could in theory mean more of its tough side effects, things like nausea, vomiting, fatigue, poor appetite, and low blood counts.
Now the reassuring part. In actual human studies, drinking pomegranate juice didn't meaningfully change levels of other CYP3A4 drugs, so this concern stays mostly theoretical. Everyday pomegranate or juice is unlikely to be a problem. Still, because Selinexor is a serious cancer medicine, it's worth a quick word with your oncologist or pharmacist before using concentrated pomegranate extracts.

What Is Selinexor Used For?
Brand name: Xpovio
Xpovio (selinexor) is used to treat multiple myeloma in adults. When used together with bortezomib and dexamethasone, it is for adults who have had at least one prior treatment. This combination is sometimes called XVd.
Xpovio is also used with dexamethasone alone (called Xd) for adults with relapsed or refractory multiple myeloma — meaning the cancer came back or stopped responding — who have already tried at least four prior therapies and whose disease no longer responds to several other standard drug types.
