Rifapentine falls under ANTIBIOTIC DRUGS — the group Probiotics interacts with.

Theoretically, taking antibiotic drugs with certain probiotics might decrease the effectiveness of probiotics.
Many probiotics are bacteria. Since these preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of sensitive organisms. Human research shows that taking probiotic concurrently with antibiotic treatment does not affect the diversity of the gut microbiome compared with antibiotic treatment in the absence of probiotics. Tell patients to separate administration of antibiotics and these preparations by at least two hours.

Here's the simple version: rifapentine (Priftin) is a powerful antibiotic that fights TB, but it doesn't tell the difference between harmful bacteria and the friendly bacteria in your probiotic. Since most probiotics are live microbes, taking them at the same time as rifapentine may kill off a good chunk of those organisms before they ever get to work. So this isn't about rifapentine becoming weaker or stronger, your TB treatment keeps doing its job. It's the probiotic that may lose some of its punch.

This is mostly a theory based on how the two work, and interestingly, some human studies haven't shown a big difference. A safe, easy fix is spacing them out by at least two hours. Run it by your pharmacist to be sure.

About Rifapentine

What Is Rifapentine Used For?

Brand name: Priftin

Rifapentine (brand name Priftin) is used to treat two forms of tuberculosis (TB) caused by Mycobacterium tuberculosis. For active pulmonary TB — an active infection in the lungs — it is approved for patients 12 years and older, always used alongside other TB medicines, never alone. For latent TB infection (LTBI) — where the bacteria are in the body but not causing active illness — it is approved for patients 2 years and older who are at high risk of the infection becoming active, always combined with isoniazid.

Priftin is not appropriate for every TB situation. It should not be used alone at any stage of active TB treatment, and it is not recommended as a once-weekly continuation regimen combined with isoniazid in people living with HIV who have active pulmonary TB, due to a higher risk of treatment failure or relapse.
