Probiotics and Linezolid

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTIBIOTIC DRUGS

Why this applies to Linezolid

Linezolid falls under ANTIBIOTIC DRUGS — the group Probiotics interacts with.

Theoretically, taking antibiotic drugs with certain probiotics might decrease the effectiveness of probiotics.
Many probiotics are bacteria. Since these preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of sensitive organisms. Human research shows that taking probiotic concurrently with antibiotic treatment does not affect the diversity of the gut microbiome compared with antibiotic treatment in the absence of probiotics. Tell patients to separate administration of antibiotics and these preparations by at least two hours.
Severity Moderate Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to keep straight: this interaction goes one direction only. Linezolid (Zyvox) is an antibiotic, and many probiotics are live bacteria. If you swallow your probiotic at the same time as your linezolid, the antibiotic can simply kill off a good chunk of those friendly organisms before they get a chance to work. So it's the probiotic that may lose its punch, not the linezolid. Your infection treatment stays just as strong.

The good news is this is a theoretical concern based on how these products behave, and human studies haven't shown a big problem. A simple fix helps: space the two doses by at least two hours. If you're taking probiotics to ease antibiotic-related diarrhea, ask your pharmacist about timing and strain.

From our pharmacist’s overview on the Linezolid page

“Linezolid (Zyvox) fights serious infections, but it also blocks an enzyme called monoamine oxidase, which is why I treat this page with real respect. Twenty of the 65 supplements here raise serotonin, and stacking them on linezolid risks serotonin syndrome, a dangerous overload of that chemical.”
