Trospium falls under ANTICHOLINERGIC DRUGS — the group Purple Nut Sedge interacts with.

In vitro, purple nut sedge dose-dependently inhibits acetylcholinesterase (AChE). Theoretically, concurrent use of anticholinergic drugs and purple nut sedge might decrease the effectiveness of purple nut sedge or the anticholinergic agent.
Some anticholinergic drugs include atropine, benztropine (Cogentin), biperiden (Akineton), procyclidine (Kemadrin), and trihexyphenidyl (Artane).

Here's the simple version for trospium (Sanctura). Trospium calms an overactive bladder by blocking a nerve signal called acetylcholine, which is what relaxes those twitchy bladder muscles. Purple nut sedge, at least in test-tube studies, appears to do the opposite by raising acetylcholine levels (it blocks the enzyme that breaks that chemical down). So in theory the two could work against each other, possibly making your trospium a little less effective and letting that urgency and frequency creep back.

I want to be honest about how soft this evidence is. It comes only from lab dishes, not from people, so we really don't know if it matters in real life. Trospium is not a prodrug, so there's no flip here. If you'd like to try the herb, just run it past your pharmacist or doctor first.

Trospium — available as tablets and extended-release capsules — is used to treat overactive bladder (OAB). It targets three main symptoms: the sudden, hard-to-control urge to urinate (urgency), leaking urine when that urge hits (urge urinary incontinence), and needing to urinate much more often than usual (urinary frequency).
