Quercetin can modestly decrease blood pressure in people with mild hypertension. Theoretically, it might have additive blood pressure lowering effects when used with antihypertensive drugs.

Here's the simple version: Hyzaar combines two blood pressure medicines, losartan (which relaxes your blood vessels) and hydrochlorothiazide (a water pill). Quercetin, in supplement doses, can nudge blood pressure down a little on its own. So when you stack it on top of Hyzaar, the two can add up and push your pressure a bit lower than expected.
If that happens, you might feel dizzy, lightheaded, or wobbly, especially when you stand up quickly, and you may feel more tired. Keep in mind this is mostly theoretical and comes from weaker studies, so the effect is usually modest, not dramatic. The small amounts of quercetin in foods like apples and onions aren't the concern; concentrated supplements are. Before starting a quercetin supplement, just check with your pharmacist or doctor.

ORGANIC ANION TRANSPORTER 3 (OAT3) SUBSTRATES

In vitro research shows that quercetin is a strong non-competitive inhibitor of OAT3, with half-maximal inhibitory concentration (IC50) values as low as 0.75 mcM. So far, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Hyzaar, which is a combo of hydrochlorothiazide (a water pill) and losartan (a blood pressure med). Your kidneys use a little transporter called OAT3 to help clear these drugs out. In lab dish studies, quercetin blocks that transporter. If that same effect happened in people, it could slow the removal of these medicines and let a bit more linger in your system.
In plain terms, that might nudge their effects up, so you could see more lightheadedness, dizziness, or low blood pressure. But honestly, this has only been seen in test tubes, never in actual patients, so it's theoretical. Food amounts of quercetin almost certainly don't matter. If you're considering high-dose supplements, just run it by your pharmacist or doctor first.

Here's the gist with Hyzaar (which combines hydrochlorothiazide and losartan) and quercetin. Your kidneys use a transporter called OAT1 to help pump these medicines out of your body. If that same effect happened in people, a little more of the drug could linger, which in theory could nudge up its effects and side effects, things like extra dizziness, low blood pressure, dehydration, or shifts in potassium and sodium.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the wrinkle with Hyzaar (which combines losartan and hydrochlorothiazide): losartan is actually a prodrug. Your body uses an enzyme called CYP3A4 (along with another, 2C9) to turn it into its more active form that lowers blood pressure. Quercetin can slow that enzyme down. In theory, that could mean less of the active losartan gets made, so your blood pressure medicine might not work quite as well.
Now, the honest part: this is mostly based on lab and animal studies, not solid human data, and food amounts of quercetin are very unlikely to matter. High-dose supplements are the bigger question mark. If you're considering quercetin capsules, just check with your pharmacist or doctor first, and keep an eye on your home blood pressure readings.

LOSARTAN (Cozaar)
5 of 5Animal research shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) while decreasing plasma levels of losartan's active metabolite. This metabolite, which is around 10-fold more potent than losartan, is the result of cytochrome P450 (CYP) 2C9- and CYP3A4-mediated transformation of losartan. Additionally, in vitro research shows that quercetin may inhibit P-glycoprotein-mediated efflux of losartan from the intestines, resulting in increased absorption of losartan. These results suggest that concomitant use of quercetin and losartan might increase systemic exposure to losartan while also decreasing plasma concentrations of losartan's active and more potent metabolite.

Here's an interesting wrinkle with Hyzaar, which combines losartan with hydrochlorothiazide. Losartan is actually a bit of a two-part drug. Your liver converts some of it into an active metabolite that's far stronger at lowering blood pressure than losartan itself. Quercetin may slow that conversion (through CYP2C9 and CYP3A4) while also helping more losartan get absorbed from your gut. The net effect is uncertain: you'd have more losartan floating around, but less of the potent metabolite that does most of the work.
This is theoretical and based only on lab and animal studies, so I wouldn't lose sleep over it. Food amounts of quercetin are very unlikely to matter. If you take high-dose supplements, just mention it to your pharmacist so your blood pressure can be watched. Don't change anything on your own.
