Levofloxacin (ophthalmic) falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP) — the group Quercetin interacts with.

Theoretically, concomitant use might increase the effects and adverse effects of OATP substrates.

In vitro evidence shows that quercetin can inhibit organic anion-transporting peptide (OATP) 1B1-mediated uptake of estrone-3-sulfate and pravastatin. Furthermore, clinical research in healthy males shows that intake of quercetin along with pravastatin increases the AUC of pravastatin by 24%, prolongs its half-life by 14%, and decreases its apparent clearance by 18%, suggesting that quercetin modestly inhibits the uptake of pravastatin in hepatic cells.

Here's the good news: the version you're asking about is levofloxacin eye drops, used right on the surface of the eye for infections. The interaction on file is about quercetin gently slowing a liver transporter (called OATP) that helps move certain medicines through the body. That kind of effect only matters when a drug is absorbed into your bloodstream in meaningful amounts.

With eye drops, almost none of the drug reaches your blood, so quercetin supplements are very unlikely to cause any real problem here. The concern applies far more to swallowed levofloxacin tablets. This link is also mostly theoretical for this drug. Still, it never hurts to mention your supplements to your pharmacist so they have the full picture.

Here's the idea behind this pairing. Levofloxacin, whether it's the eye drop or the tablet, kills bacteria by locking onto an enzyme called DNA gyrase. In lab dish studies, quercetin can grab onto that very same spot on the bacteria, which could theoretically get in the way and make the antibiotic a little less effective at clearing your infection.

I want to be honest that this is mostly test-tube science, not something well proven in people, and it applies to swallowed antibiotics. With levofloxacin eye drops, the medicine works right on the surface of your eye and very little gets into your bloodstream, so a quercetin capsule is unlikely to reach your eye and interfere. Still, it never hurts to run your supplement list by your pharmacist or doctor to be safe.

Brand name: Levofloxacin

Levofloxacin tablets, oral solution and injection treat infections caused by susceptible bacteria in adults. These include several types of pneumonia, skin infections, urinary tract infections, chronic bacterial prostatitis, bronchitis flare-ups and sinus infections. They also include post-exposure treatment for inhalational anthrax and plague.

Levofloxacin tablets are also used in children for inhalational anthrax (post-exposure) and plague. The eye drops are different: they treat bacterial conjunctivitis or corneal ulcer, depending on the product. It only works on bacteria, not viruses.
