Why this applies to Mometasone, formoterol

Mometasone, formoterol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Rabdosia Rubescens interacts with.

In vitro research shows that oridonin, a constituent of Rabdosia rubescens, induces cytochrome P450 3A4 (CYP3A4) enzyme. Also, in an animal model, oridonin induces a gene homologous to CYP3A4. So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking Rabdosia rubescens. Some drugs metabolized by CYP3A4 that might be affected are some benzodiazepines such as alprazolam (Xanax), diazepam (Valium), midazolam (Versed), triazolam (Halcion); calcium channel blockers such as amlodipine (Norvasc), diltiazem (Cardizem, Dilacor, others), felodipine (Plendil), verapamil (Calan, Verelan, others); cyclosporine (Neoral, Sandimmune); some HIV antivirals such as indinavir (Crixivan), ritonavir (Norvir), saquinavir (Fortovase, Invirase); some HMG CoA reductase inhibitors such as atorvastatin (Lipitor), lovastatin (Mevacor); some macrolide antibiotics such as erythromycin and clarithromycin (Biaxin); quinidine; and many others.

Your Zenhale inhaler combines two medicines: mometasone, an inhaled steroid that calms airway swelling, and formoterol, which opens up your airways. The steroid part is one of the medicines broken down by a liver enzyme called CYP3A4. Lab and animal studies suggest a compound in Rabdosia rubescens (oridonin) may speed up that enzyme, which in theory could clear the mometasone a little faster and make it slightly less effective at controlling your asthma over time.

Here's my honest take: this is only test-tube and animal evidence, it has never been shown in real people, and inhaled steroids act mostly right in your lungs. So I wouldn't lose sleep over it. Still, if your breathing or asthma control feels worse, loop in your pharmacist or doctor before changing anything.

Mometasone, formoterol also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

In vitro research shows that oridonin, a constituent of Rabdosia rubescens, induces cytochrome P450 2C9 (CYP2C9) enzyme. So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2C9 in patients taking Rabdosia rubescens. Some drugs metabolized by CYP2C9 include nonsteroidal anti-inflammatory drugs (NSAIDs) such as diclofenac (Cataflam, Voltaren), ibuprofen (Motrin), meloxicam (Mobic), and piroxicam (Feldene); celecoxib (Celebrex); amitriptyline (Elavil); warfarin (Coumadin); glipizide (Glucotrol); losartan (Cozaar); and others.

Here's the honest picture with Zenhale (mometasone and formoterol). The concern on file is that oridonin, a compound in Rabdosia rubescens, might speed up a liver enzyme called CYP2C9. In theory, a faster enzyme could clear certain drugs more quickly and make them a little weaker. But this comes from test-tube research only, it has never been shown in real people, and it's rated a minor interaction.

On top of that, mometasone and formoterol aren't mainly handled by CYP2C9, and they're inhaled into your lungs rather than swallowed, so very little reaches your liver. I wouldn't expect your asthma or COPD control to change here. Still, since this herb hasn't been well studied for safety, run it past your pharmacist or doctor before starting it.
