Perampanel falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Rabdosia Rubescens interacts with.

In vitro research shows that oridonin, a constituent of Rabdosia rubescens, induces cytochrome P450 3A4 (CYP3A4) enzyme. Also, in an animal model, oridonin induces a gene homologous to CYP3A4. So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking Rabdosia rubescens. Some drugs metabolized by CYP3A4 that might be affected are some benzodiazepines such as alprazolam (Xanax), diazepam (Valium), midazolam (Versed), triazolam (Halcion); calcium channel blockers such as amlodipine (Norvasc), diltiazem (Cardizem, Dilacor, others), felodipine (Plendil), verapamil (Calan, Verelan, others); cyclosporine (Neoral, Sandimmune); some HIV antivirals such as indinavir (Crixivan), ritonavir (Norvir), saquinavir (Fortovase, Invirase); some HMG CoA reductase inhibitors such as atorvastatin (Lipitor), lovastatin (Mevacor); some macrolide antibiotics such as erythromycin and clarithromycin (Biaxin); quinidine; and many others.

Here's the gist for Perampanel (Fycompa). Your body relies heavily on a liver enzyme called CYP3A4 to clear this seizure medicine. Lab and animal studies hint that oridonin, a compound in Rabdosia rubescens, may speed up (induce) that enzyme. If that happens in people, your perampanel could get broken down faster and drop to lower levels, which may mean weaker seizure control. That matters, because the goal here is keeping seizures away.

I want to be honest about the evidence: this is only test-tube and animal data, and it hasn't been confirmed in humans. So don't panic, and please don't change anything on your own. Just check with your pharmacist or doctor before adding this herb, so they can keep an eye on your seizures.

Perampanel also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

In vitro research shows that oridonin, a constituent of Rabdosia rubescens, induces cytochrome P450 2C9 (CYP2C9) enzyme. So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2C9 in patients taking Rabdosia rubescens. Some drugs metabolized by CYP2C9 include nonsteroidal anti-inflammatory drugs (NSAIDs) such as diclofenac (Cataflam, Voltaren), ibuprofen (Motrin), meloxicam (Mobic), and piroxicam (Feldene); celecoxib (Celebrex); amitriptyline (Elavil); warfarin (Coumadin); glipizide (Glucotrol); losartan (Cozaar); and others.

Here's the gist for Fycompa (perampanel): this herb contains a compound called oridonin, and in test-tube studies it seemed to speed up a liver enzyme (CYP2C9) that helps clear certain medicines. If that same effect happened in your body, your seizure medicine could be broken down a little faster, meaning lower levels and possibly less seizure protection.

That said, I don't want to overstate this. The finding comes only from lab research, it has never been reported in real people, and it's rated as a minor, possible interaction. Still, because we never want a seizure medicine slipping below where it works, it's worth a quick chat with your doctor or pharmacist before adding Rabdosia rubescens. They can watch for any change in seizure control.

What Is Perampanel Used For?

Brand name: Fycompa

Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.

Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
