These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Rauwolscine interacts with whole categories of medication. Acetaminophen, Chlorpheniramine, Dextromethorphan falls into 2 of those categories, so it can interact with Rauwolscine in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
SEIZURE THRESHOLD LOWERING DRUGS
1 of 2Why this applies to Acetaminophen, Chlorpheniramine, Dextromethorphan
Acetaminophen, Chlorpheniramine, Dextromethorphan falls under SEIZURE THRESHOLD LOWERING DRUGS — the group Rauwolscine interacts with.
In animal research, intraperitoneal rauwolscine lowered the seizure threshold level of the drug metrazol.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with this combo cold medicine (the Coricidin product with acetaminophen, chlorpheniramine, and dextromethorphan). Rauwolscine is a strong stimulant alkaloid, and in animal studies it lowered the threshold for seizures. A few ingredients here can nudge that same direction in sensitive people. Both dextromethorphan and the antihistamine chlorpheniramine can, in high doses or in folks prone to seizures, make seizures a bit more likely, so stacking them with a seizure-lowering stimulant is the theoretical worry.
To be fair, this is based on lab and animal data only, not real patient cases, so for most healthy people the actual risk is low. Still, you might notice jitteriness, a racing heart, or anxiety from the combo. If you have any seizure history or heart issues, please check with your pharmacist or doctor before mixing these.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
2 of 2Acetaminophen, Chlorpheniramine, Dextromethorphan also falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES.
Rauwolscine is structurally related to yohimbine. In vitro research shows that yohimbine inhibits CYP2D6 enzyme activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This cold and flu combo contains dextromethorphan (the cough suppressant), which your body clears using an enzyme called CYP2D6. Rauwolscine is a cousin of yohimbine, and in lab tests yohimbine has slowed that enzyme down. If that holds true in people, dextromethorphan could build up a bit more than usual, which might mean extra drowsiness, dizziness, or in higher amounts, feeling agitated or restless. The chlorpheniramine in this product can already make you sleepy, so the two together could add up.
I want to be honest though: this is a theoretical concern based only on lab data, not real patient cases. It may never cause a problem. Still, since rauwolscine itself can raise heart rate and blood pressure, it's worth running this combo past your pharmacist before mixing them.

From our pharmacist’s overview on the Acetaminophen, Chlorpheniramine, Dextromethorphan page
“Alcohol is the risk to understand first. It carries major ratings alongside the acetaminophen, chlorpheniramine, and dextromethorphan combination sold as Coricidin II Extra Strength Cold and Flu, because acetaminophen already taxes the liver, and heavy drinking on top of it raises the chance of genuine liver injury.”
