Codeine Phosphate falls under CNS DEPRESSANTS — the group Sage interacts with.
Some constituents of sage have CNS depressant activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version. Codeine is an opioid that can already make you drowsy, foggy, and slow your breathing. Sage contains some natural compounds that, in lab and animal studies, seem to have a mild calming or sedating effect of their own. The worry is that putting the two together could add up, so you might feel sleepier or more lightheaded than codeine alone would cause.
Keep in mind this is theory, not proven in people. The evidence comes only from test-tube and animal work, and a pinch of sage on your dinner is very unlikely to matter. Concentrated sage extracts or the essential oil are a different story. If you take codeine regularly and want to use sage supplements, just run it by your pharmacist or doctor first.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

In vitro evidence suggests that aqueous extracts of sage can inhibit CYP2D6. So far, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the twist with codeine: it's actually a prodrug. Your body has to convert it into morphine using an enzyme called CYP2D6 before you get much pain relief. Lab studies suggest sage may slow that same enzyme down. If that happened in real life, you might convert less codeine into its active form, which could mean weaker pain or cough relief rather than more side effects.
I want to be honest about how shaky this is. The evidence comes only from test-tube studies, and it has never actually been reported in people. Cooking with sage almost certainly won't matter; concentrated extracts or sage oil are where I'd be more cautious. If you take codeine regularly and want to use sage supplements, just run it by your pharmacist or doctor first.
