Methyldopa falls under ANTIHYPERTENSIVE DRUGS — the group Sage interacts with.

Theoretically, sage might increase or decrease the effects of antihypertensive drugs.
Animal research suggests that common sage (Salvia officinalis) can cause prolonged blood pressure reduction. However, clinical research suggests that Spanish sage (Salvia lavandulaefolia) can increase blood pressure in some people with hypertension. Until more is known, use with caution.

Here's the honest picture with sage and your blood pressure medicine, methyldopa. This interaction is mostly theoretical and comes from lab and animal studies, so it isn't something proven to happen in real patients. The tricky part is that sage could pull in two different directions depending on the type. Common sage (Salvia officinalis) may lower blood pressure further, while Spanish sage (Salvia lavandulaefolia) has actually raised blood pressure in some folks.

So if you take concentrated sage extracts or the essential oil along with methyldopa, your blood pressure could drift either too low (leaving you dizzy, lightheaded, or tired) or harder to control. The sage you sprinkle on food is very unlikely to cause any of this. If you're thinking about supplement-strength sage, just run it by your pharmacist or doctor first.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory.
