Why this applies to Estradiol, Progesterone

Estradiol, Progesterone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Schisandra interacts with.

Schisandra can increase the levels and clinical effects of drugs metabolized by CYP3A4.
Most clinical and laboratory research shows that schisandra, administered either as a single dose or up to twice daily for 14 days, inhibits CYP3A4 and increases the concentration of CYP3A4 substrates such as cyclophosphamide, midazolam, tacrolimus, and talinolol. Although one in vitro and animal study shows that schisandra may induce CYP3A4 metabolism, this effect appears to be overpowered by schisandra's CYP3A4 inhibitory activity and has not been reported in humans.

Here's the simple version. Your Bijuva is a combination of estradiol and progesterone, and both hormones are cleared by a liver enzyme called CYP3A4. Schisandra tends to slow that enzyme down. When the enzyme works slower, these hormones can hang around longer and build up a bit, which may nudge their effects (and side effects) higher.

For you that could mean more of the usual hormone-related complaints, things like breast tenderness, bloating, nausea, headaches, mood changes, or spotting. The good news is most of the proof here comes from lab and animal work, not real patients, so this is more of a heads-up than an alarm. Still, since you'd be combining two active products, it's worth a quick chat with your pharmacist or doctor before adding schisandra.

Schisandra might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that schisandra extracts and constituents such as schisandrin B inhibit P-glycoprotein mediated efflux in intestinal cells and in P-glycoprotein over-expressing cell lines. Additionally, a small clinical study shows that schisandra increases the peak concentration and overall exposure to talinolol, a P-glycoprotein probe substrate. Theoretically, schisandra might inhibit the efflux of other P-glycoprotein substrates.

Bijuva combines estradiol and progesterone, and both of these hormones are handled in part by a transporter pump called P-glycoprotein. Schisandra appears to slow this pump down, at least in lab studies. The idea is that if the pump works less, a little more of these hormones could get absorbed and stay in your body longer. Neither hormone is a prodrug, so the simple takeaway is that schisandra might gently nudge their levels up.

In real life this could mean more of the usual hormone effects, like breast tenderness, bloating, nausea, headaches, or mood changes. Still, this is based mostly on lab and animal work, so it is more theoretical than proven. I would not worry, but do mention schisandra to your pharmacist or doctor before combining them.
