Schisandra and Ethinyl Estradiol, Norethindrone, Ferrous Fumarate

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Schisandra interacts with whole categories of medication. Ethinyl Estradiol, Norethindrone, Ferrous Fumarate falls into 2 of those categories, so it can interact with Schisandra in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

1 of 2

Why this applies to Ethinyl Estradiol, Norethindrone, Ferrous Fumarate

Ethinyl Estradiol, Norethindrone, Ferrous Fumarate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Schisandra interacts with.

Schisandra can increase the levels and clinical effects of drugs metabolized by CYP3A4.
Most clinical and laboratory research shows that schisandra, administered either as a single dose or up to twice daily for 14 days, inhibits CYP3A4 and increases the concentration of CYP3A4 substrates such as cyclophosphamide, midazolam, tacrolimus, and talinolol. Although one in vitro and animal study shows that schisandra may induce CYP3A4 metabolism, this effect appears to be overpowered by schisandra's CYP3A4 inhibitory activity and has not been reported in humans.
Severity Moderate Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Your birth control pill contains two hormones, ethinyl estradiol and norethindrone, and both are cleared from your body by a liver enzyme called CYP3A4. Schisandra appears to slow that enzyme down. In plain terms, that could let a little more estrogen and progestin linger in your bloodstream than usual. You might notice more of the typical hormone-related effects, like nausea, breast tenderness, headaches, bloating, or breakthrough spotting.

I want to be honest about the evidence here: this comes mostly from lab and animal studies, not solid human research, so it's more of a theoretical concern than a proven problem. The iron (ferrous fumarate) part isn't involved. If you'd like to use schisandra, just run it by your pharmacist or doctor first so you can keep an eye on things together.

P-GLYCOPROTEIN SUBSTRATES

2 of 2

Ethinyl Estradiol, Norethindrone, Ferrous Fumarate also falls under P-GLYCOPROTEIN SUBSTRATES.

Schisandra might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that schisandra extracts and constituents such as schisandrin B inhibit P-glycoprotein mediated efflux in intestinal cells and in P-glycoprotein over-expressing cell lines. Additionally, a small clinical study shows that schisandra increases the peak concentration and overall exposure to talinolol, a P-glycoprotein probe substrate. Theoretically, schisandra might inhibit the efflux of other P-glycoprotein substrates.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the short version on combining Schisandra with your birth control pill (the estrogen plus progestin combo in Loestrin, Microgestin, Junel, and similar brands). Schisandra appears to block a transporter called P-glycoprotein, which normally pumps certain drugs back out of your gut. When that pump is slowed, a little more of the hormone may get absorbed. In theory, that could nudge your hormone levels up and slightly raise the chance of side effects like nausea, breast tenderness, headaches, or breakthrough spotting.

I want to be honest about the evidence here, though. It comes mostly from lab and animal studies plus one small human study using a different drug, so this is largely theoretical. It's not a reason to panic, but it's worth a quick chat with your pharmacist or doctor before starting Schisandra.

About Schisandra

What is Schisandra?

Schisandra
  • Fatigue and low energy
  • Stress and adaptogen support
  • Liver health
  • Mental focus and performance
  • Menopause symptoms
  • Respiratory complaints

Schisandra is a traditional Chinese medicine berry used as an adaptogen for stress, fatigue, and liver support. Human evidence is limited and most claims are not well proven, but it appears reasonably well tolerated by most healthy adults when used short term. Pregnant and breastfeeding people should avoid it, and anyone with a health condition should check with a pharmacist or doctor first.
