Why this applies to Riociguat

Riociguat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Schisandra interacts with.

Schisandra can increase the levels and clinical effects of drugs metabolized by CYP3A4.
Most clinical and laboratory research shows that schisandra, administered either as a single dose or up to twice daily for 14 days, inhibits CYP3A4 and increases the concentration of CYP3A4 substrates such as cyclophosphamide, midazolam, tacrolimus, and talinolol. Although one in vitro and animal study shows that schisandra may induce CYP3A4 metabolism, this effect appears to be overpowered by schisandra's CYP3A4 inhibitory activity and has not been reported in humans.

Here's the simple version: your body relies on a liver enzyme called CYP3A4 to break down and clear riociguat. Schisandra tends to slow that enzyme down. When that happens, riociguat isn't cleared as quickly, so more of it can build up in your blood than expected.

With riociguat, higher levels usually mean a stronger blood-pressure-lowering effect. That can show up as dizziness, lightheadedness, headache, low blood pressure, or fainting, especially when you stand up. Most of the evidence here comes from lab and animal work rather than people taking these two together, so this is more of a caution than a certainty. Still, riociguat is a medication where careful dosing matters, so please check with your pharmacist or doctor before adding schisandra. Don't change your riociguat on your own.

Schisandra might increase the levels and clinical effects of P-glycoprotein substrates.
In vitro research shows that schisandra extracts and constituents such as schisandrin B inhibit P-glycoprotein mediated efflux in intestinal cells and in P-glycoprotein over-expressing cell lines. Additionally, a small clinical study shows that schisandra increases the peak concentration and overall exposure to talinolol, a P-glycoprotein probe substrate. Theoretically, schisandra might inhibit the efflux of other P-glycoprotein substrates.

Here's the gist for Adempas (riociguat). This medicine is moved around your body partly by a transporter called P-glycoprotein, which acts like a doorman pushing the drug back out of your cells. Schisandra appears to slow that doorman down, so more riociguat could stay in your bloodstream than usual.

Since riociguat works by relaxing blood vessels, higher levels could nudge its effects too far. That might mean more low blood pressure, dizziness, lightheadedness, headache, or feeling faint when you stand up. The honest part: this concern is based mostly on lab and animal work plus one small study with a different drug, so it's a theoretical caution, not a proven problem. Because riociguat already needs careful blood pressure monitoring, please check with your pharmacist or doctor before pairing it with Schisandra.

About Riociguat

What Is Riociguat Used For?

Brand name: Adempas

Riociguat tablets — sold under the brand names Adempas and Riociguat — are approved for adults with two types of high blood pressure in the lungs. The first is chronic thromboembolic pulmonary hypertension (CTEPH), a condition caused by blood clots that have blocked or narrowed the arteries in the lungs. Riociguat is used when surgery has not fully fixed the problem, or when surgery is not an option.

The second is pulmonary arterial hypertension (PAH), a progressive disease where the arteries in the lungs become stiff and narrowed. In PAH, riociguat helps improve exercise ability, functional class (how well you can get around day-to-day), and slows disease worsening. It can be used alone or combined with other PAH medicines called endothelin receptor antagonists or prostanoids.
