Fentanyl citrate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Schizonepeta interacts with.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, inhibits cytochrome P450 (CYP) 2D6. Theoretically, schizonepeta might increase the effects and side effects of CYP2D6 substrates.
Some substrates of CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), haloperidol (Haldol), imipramine (Tofranil), metoprolol (Lopressor, Toprol XL), ondansetron (Zofran), paroxetine (Paxil), risperidone (Risperdal), tramadol (Ultram), venlafaxine (Effexor), and others.

Here's the thing about pairing Schizonepeta with fentanyl: the concern on file is that a compound in this herb may block a liver enzyme called CYP2D6. In theory, blocking that enzyme could let certain drugs build up and hit harder. With fentanyl, though, I want to keep this in perspective. Fentanyl is mainly broken down by a different enzyme (CYP3A4), so CYP2D6 plays only a minor role in clearing it. That makes a meaningful interaction less likely than this category label suggests.

Also worth noting, this warning comes from lab and animal research only, not human studies, so it's pretty theoretical. Still, fentanyl is a powerful opioid where extra drowsiness or slowed breathing is serious. Please loop in your doctor or pharmacist before combining the two.

Animal research suggests that schizonepetin, a monoterpene constituent of schizonepeta, induces cytochrome P450 (CYP) 3A4. Theoretically, schizonepeta might decrease the effects of CYP3A4 substrates.
Some substrates of CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the short version: fentanyl is a powerful opioid pain medicine, and your body clears it using a liver enzyme called CYP3A4. Lab and animal studies hint that a compound in Schizonepeta may speed up that enzyme. If that happened in people, your body could break fentanyl down a little faster, which might make it work less well, so you could get less pain control than expected.

I want to be honest about how thin this evidence is. It comes only from test tube and animal research, not from real patients, so this is theoretical. Fentanyl is still a serious, high-risk drug, so please don't add or change anything on your own. Run it by your doctor or pharmacist first.
