Here is the interaction we found between these two, with the evidence behind it.
ANTIDIABETES DRUGS
Why this applies to Empagliflozin
Empagliflozin falls under ANTIDIABETES DRUGS — the group Sesame interacts with.
Clinical studies show that sesame oil can decrease plasma glucose and glycated hemoglobin (HbA1c) levels. Some clinical research in patients taking glibenclamide shows that using sesame oil or a blend of sesame oil and rice bran oil in place of other oil for cooking reduces plasma glucose more than glibenclamide alone. Monitor blood glucose levels closely. Dose adjustments might be necessary.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: sesame, especially as a cooking oil, may have a mild blood-sugar-lowering effect of its own. Since Jardiance (empagliflozin) already works to bring your glucose down (it makes your kidneys flush out extra sugar in your urine), stacking the two could nudge your numbers a little lower than expected. In theory that raises the odds of low blood sugar, with signs like shakiness, sweating, dizziness, or feeling foggy, though empagliflozin on its own rarely causes serious lows unless you're also on insulin or a sulfonylurea.
Honestly, this is based mostly on pharmacology and small studies, so I wouldn't lose sleep over a little sesame on your salad or stir-fry. If you're using concentrated sesame oil daily, just keep an eye on your glucose readings and mention it to your pharmacist or doctor before changing anything.

From our pharmacist’s overview on the Empagliflozin page
“Almost everything on this page can nudge blood sugar in someone taking empagliflozin (Jardiance) for type 2 diabetes, heart failure, or kidney protection, and two entries deserve genuine caution before anything else. Siberian cocklebur has caused severe low blood sugar in humans and should not be combined with any glucose-lowering medi…”

About Sesame
What is Sesame?
- Heart and cholesterol health
- Blood pressure support
- Source of healthy fats and minerals
- Skin and hair care (topical oil)
- Antioxidant support
- Cooking and nutrition
Sesame is a nutritious seed and oil widely used in cooking and traditional medicine, and it provides healthy fats, fiber, and antioxidant compounds called lignans. Some early research suggests sesame may modestly help with cholesterol and blood pressure, but the evidence is limited. Sesame is also a common, sometimes serious, food allergen, so people with seed allergies should be careful.
About Empagliflozin
What Is Empagliflozin Used For?
Brand name: Jardiance
Jardiance (empagliflozin) tablets are approved for several important uses in adults. It reduces the risk of cardiovascular death and hospitalization in adults with heart failure. It also slows the progression of chronic kidney disease by reducing the risk of serious kidney decline, end-stage kidney disease, cardiovascular death, and hospitalization. In adults with type 2 diabetes and established cardiovascular disease, Jardiance reduces the risk of cardiovascular death.
Jardiance is also used — alongside diet and exercise — to improve blood sugar control in type 2 diabetes in adults and children aged 10 and older. It is not recommended for blood sugar control in type 1 diabetes (it may raise the risk of a dangerous condition called diabetic ketoacidosis) or in type 2 diabetes patients with severely reduced kidney function.

Empagliflozin Is Linked to Lower Levels of One Nutrient
Natural Medicines records Empagliflozin as a medicine that can lower these. This is not an interaction with Sesame — it is a separate effect of the medicine, and it is a reason some people are advised to supplement rather than to avoid one:
