Opicapone falls under P-GLYCOPROTEIN SUBSTRATES — the group Sesame interacts with.

Theoretically, sesame might alter the transport of P-glycoprotein substrates.
In vitro research suggests that sesamin, a constituent of sesame, can inhibit the multi-drug transporter protein, P-glycoprotein. However, this interaction has not been reported in humans.

Here's the gist with sesame and opicapone (Ongentys). Lab studies hint that sesamin, one of the natural compounds in sesame, may gently block a transporter called P-glycoprotein. Opicapone happens to be moved around by that same transporter, so in theory, blocking it could let a little more opicapone stay in your system.

If that happened, you might notice more of opicapone's usual effects, like extra involuntary movements (dyskinesia), low blood pressure when standing, or trouble sleeping. That said, this is only seen in test-tube research, never in real people, and the eating amounts of sesame in your meals are very unlikely to matter. Concentrated sesame supplements are less studied. If you take those, just mention it to your pharmacist or doctor.

Ongentys (opicapone) is used alongside levodopa/carbidopa to treat Parkinson's disease in adults who experience 'off' episodes — periods when medication wears off and Parkinson's symptoms return before the next dose is due.

It is not used alone. Ongentys works as an add-on to help levodopa stay active longer, reducing the time patients spend in these 'off' states.
