This combination pill contains ergotamine (for migraines), belladonna, and phenobarbital. The piece that matters most here is ergotamine, which your liver clears using an enzyme called CYP3A4. St. John's Wort speeds that enzyme up, so it can push the ergotamine out of your system faster and lower its levels. In plain terms, your migraine relief may simply not work as well as it should.

The effect on phenobarbital can go a similar direction, possibly making seizure or sedative control less reliable. The evidence here is moderate, but I'd treat it seriously. Please don't change anything on your own. Just check with your pharmacist or doctor before pairing these, so they can keep your treatment steady.

Here's the thing to know about combining St. John's wort with this combination product, which contains phenobarbital. St. John's wort revs up the liver enzymes that clear phenobarbital from your body. That means your phenobarbital levels can drop, and the medicine may stop working as well as it should.

Since phenobarbital here is often helping calm symptoms (and in some people supports seizure control), weaker levels could mean a return of headaches or other symptoms the combo was managing, and lower seizure protection. Phenobarbital is one of those drugs where steady, reliable blood levels really matter, so even modest changes count. Please don't start or stop anything on your own. Talk with your pharmacist or doctor first so they can check levels and adjust safely.

This combination pill mixes ergotamine (for migraines), belladonna, and phenobarbital (a barbiturate sedative). St. John's wort tends to speed up certain liver enzymes, and the one on file here (CYP2B6) helps clear phenobarbital. In plain terms, the herb may push your body to break down the phenobarbital part a little faster, which could make it slightly less effective at calming things down.

Honestly, the bigger worry with St. John's wort and this drug runs the other way: phenobarbital itself is a strong enzyme booster, and the two together can affect each other in unpredictable ways. The measured changes here were modest (around 14 to 20 percent in a similar drug), so I wouldn't panic.
