PHENOBARBITAL (Luminal)
1 of 2St. John's wort may increase the metabolism of phenobarbital. Plasma concentrations of phenobarbital should be monitored carefully. The dose of phenobarbital may need to be increased when St. John's wort is started and decreased when it is stopped.
Well-controlled human studies have demonstrated the likely existence of this interaction.
This combination product pairs hyoscyamine with phenobarbital, and it's the phenobarbital part that matters here. St. John's wort revs up the liver enzymes that clear phenobarbital out of your body. The result is that your phenobarbital levels can drop, and the medicine may not work as well as it should.
Phenobarbital is a barbiturate with a narrow safety window, so even modest changes in its level are a real concern. If it falls too low, you could lose seizure control or whatever calming effect you're relying on it for. And if you stop the St. John's wort, levels can swing back up and cause more drowsiness or grogginess. Please don't start or stop either one on your own. Talk with your pharmacist or doctor first so they can monitor your levels and adjust safely.

Clinical research shows that taking St. John's wort 325 mg three times daily for 14 days along with bupropion, a CYP2B6 substrate, reduces the area under the concentration-time curve by approximately 14% and increases the clearance of bupropion by approximately 20%.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with this combo. Your medicine pairs hyoscyamine (helps with cramping and stomach issues) with phenobarbital, a barbiturate that calms things down. St. John's wort tends to speed up certain liver enzymes, so it can push your body to clear phenobarbital a little faster. In plain terms, the calming, sedating effect of the phenobarbital portion may end up weaker than expected.
The research behind this is modest (a roughly 14 to 20 percent shift seen with a similar drug, not phenobarbital itself), so don't panic. But phenobarbital is one of those medicines where steady levels matter. Please loop in your pharmacist or doctor before adding St. John's wort so they can keep an eye on your symptoms.
