Larotrectinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sulforaphane interacts with.

Theoretically, sulforaphane might increase the levels and effects of CYP3A4 substrates.
Animal and in vitro research shows that sulforaphane inhibits CYP3A4 enzyme activity and downregulates its expression. This interaction has not been reported in humans.

Here's the quick picture: your body uses an enzyme called CYP3A4 to break down and clear larotrectinib (Vitrakvi). Lab and animal studies hint that sulforaphane may slow that enzyme down. If it did the same in people, larotrectinib could clear more slowly and build up a bit, which can mean stronger effects and a higher chance of side effects like dizziness, fatigue, nausea, or changes in liver tests.

The important part: this is theoretical only, seen in test tubes and animals, and it has never been reported in humans. Eating broccoli and other cruciferous veggies is very unlikely to matter. A concentrated sulforaphane supplement is the bigger question mark, so since larotrectinib is a serious cancer drug, run any supplement past your oncology team or pharmacist first.

What Is Larotrectinib Used For?

Brand name: Vitrakvi

Vitrakvi is used to treat adult and pediatric patients with solid tumors that have a neurotrophic receptor tyrosine kinase (NTRK) gene fusion — a specific genetic alteration that can drive tumor growth. It is used when the tumor is metastatic (has spread) or when surgery to remove it would likely cause serious harm, and when there are no satisfactory alternative treatments or the cancer has continued to grow after prior treatment.

Patients must be selected for Vitrakvi therapy based on an FDA-approved test that confirms the NTRK gene fusion in their tumor. This approval is under the FDA's accelerated approval pathway, based on how many patients responded to treatment and how long those responses lasted. Continued approval may depend on results from follow-up clinical trials.
