Here's the simple version: both Sweet Annie and your Oseni (alogliptin plus pioglitazone) can, in rare cases, be tough on the liver. Pioglitazone in particular carries a known, though uncommon, risk of liver stress. The worry is that taking them together could add up and put extra strain on your liver.

I want to be honest, this is mostly a theoretical concern based on scattered reports, not strong studies, so don't panic. Still, watch for warning signs like yellowing of the skin or eyes, dark urine, unusual tiredness, nausea, or pain on your upper right side. If any of those show up, call your doctor. Before adding Sweet Annie, please check with your pharmacist or doctor, and keep up with any liver bloodwork they order.

Alogliptin, Pioglitazone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.

Here's the situation with Oseni, which combines alogliptin and pioglitazone for type 2 diabetes. The pioglitazone part is partly handled by a liver enzyme called CYP3A4, and Sweet Annie's artemisinin may nudge that enzyme one way or the other. In lab dishes, purified artemisinin sped the enzyme up (which could lower pioglitazone levels and weaken blood sugar control), but the whole-herb extract did the opposite. So the real-world direction is genuinely unclear.

Honestly, this is only test-tube evidence, so I wouldn't lose sleep over it. Still, if you add Sweet Annie, keep an eye on your blood sugar readings and check with your pharmacist or doctor first before mixing the two.
