Here's the simple version: this isn't about Sweet Annie changing how your body handles amiodarone. Instead, the worry is that both can be hard on the liver. Amiodarone is already well known for occasionally stressing liver enzymes, and there's some concern Sweet Annie (Artemisia annua) can do the same. Put them together and the effects could, in theory, stack up.

I want to be honest about the evidence here, it's mostly anecdotal, not solid proof. Still, with amiodarone I lean cautious. Watch for things like unusual tiredness, nausea, belly pain on the upper right side, dark urine, or yellowing of your skin or eyes. Please loop in your pharmacist or doctor before combining these, since your liver is likely already being monitored.

Amiodarone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.

Here's the situation with amiodarone and Sweet Annie. Your body relies on a liver enzyme called CYP3A4 to help break amiodarone down. Sweet Annie contains artemisinin, and lab studies show it can pull this enzyme in two different directions: one part of the plant seems to speed the enzyme up, while the whole extract seems to slow it down. Because the signals conflict, nobody can say for sure which way things would tip in a real person.

Why this matters: amiodarone is a tricky, narrow-margin drug that already lingers in the body for weeks. If its levels drift up, you could see more side effects like a very slow heartbeat, dizziness, or thyroid and lung trouble; if levels drop, your rhythm protection could weaken. The current evidence is only from lab dishes, so this is more of a caution than a proven problem. Please loop in your pharmacist or cardiologist before combining them.

What Is Amiodarone Used For?

Amiodarone is prescribed for adults with serious, life-threatening heart-rhythm problems that haven't responded to other medications. Specifically, it's used for two dangerous arrhythmias (abnormal heartbeats): recurrent ventricular fibrillation (a chaotic, potentially fatal heart rhythm) and recurrent ventricular tachycardia (a dangerously fast heart rhythm that causes unstable blood pressure).

Because amiodarone comes with significant risks, doctors reserve it for situations where other options haven't worked or can't be tolerated. It is not a first-choice medication — it's a last-resort tool for serious, life-threatening conditions.
