Here's the thing to keep in mind: Bosentan can be hard on your liver, which is why people on it get regular liver blood tests. Sweet Annie (Artemisia annua) has also been linked to possible liver irritation in some reports. Putting two things that may stress the liver together could, in theory, add up and raise your risk of trouble.

I'll be honest, this concern is based on anecdotal reports and theory, not strong studies, so it's not a sure thing. Still, with bosentan it's worth being cautious. Watch for warning signs like yellowing skin or eyes, dark urine, belly pain, nausea, or unusual tiredness, and don't change anything on your own. Please run Sweet Annie past your pharmacist or doctor first.

Bosentan also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.

Here's the situation with Sweet Annie and bosentan (Tracleer). Your liver uses an enzyme called CYP3A4 to clear bosentan from your body. Sweet Annie may tug on that enzyme, but the research is genuinely mixed. One component, artemisinin, seemed to speed the enzyme up in lab tests (which could lower bosentan levels and weaken its effect on your lung pressure), while the whole-plant extract appeared to slow it down (which could raise levels and bring more side effects like headache, flushing, swelling in the legs, or low blood pressure).

Bosentan is not a prodrug, so its active form is what gets cleared here. The honest takeaway is that this is theoretical, test-tube-level evidence only, not proven in people. Still, since PAH is serious, please check with your pharmacist or doctor before combining them.

What Is Bosentan Used For?

Brand name: Tracleer

Bosentan is used to treat pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs. In adults, it helps improve the ability to exercise and slows how quickly the disease gets worse.

In children aged 3 years and older with idiopathic (no known cause) or congenital (present from birth) PAH, bosentan is used to lower resistance in the lung blood vessels, which is expected to improve exercise ability. Tracleer is one brand approved for both adults and pediatric patients with PAH.
