Here's the simple version: this isn't about Sweet Annie changing how your body activates or clears Entrectinib. Instead, the worry is that both can be hard on the liver, and stacking them could add up. Entrectinib itself can raise liver enzymes and, in some people, cause more serious liver trouble. Sweet Annie has also been linked to liver irritation in a few reports.

To be honest, this concern is mostly theoretical and based on weak, anecdotal evidence, not strong studies. Still, your cancer team already watches your liver closely on this drug, so adding something that could nudge those numbers isn't worth the risk. Please check with your oncologist or pharmacist before using Sweet Annie.

Entrectinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.

Here's the situation with Sweet Annie and your entrectinib (Rozlytrek). Your body relies on a liver enzyme called CYP3A4 to clear entrectinib out of your system. Sweet Annie may tug on that same enzyme, but honestly the direction isn't clear. In lab dishes, one component (artemisinin) seems to speed the enzyme up, while the whole-plant extract seems to slow it down. So we can't say for sure which way the scale tips in a real person.

If it speeds clearance, entrectinib could become less effective. If it slows clearance, levels could climb and bring more side effects like dizziness, swelling, fatigue, or heart-rhythm changes. Since this is only lab data and entrectinib is a serious cancer drug, please loop in your oncology team before adding Sweet Annie.

What Is Entrectinib Used For?

Brand name: Rozlytrek

Rozlytrek (entrectinib) is used to treat adult patients with ROS1-positive metastatic non-small cell lung cancer (NSCLC), as confirmed by an FDA-approved test. It is also approved for adult and pediatric patients older than 1 month of age who have solid tumors — any cancer type — that carry an NTRK gene fusion detected by an FDA-approved test, provided the tumor has no known acquired resistance mutation, is metastatic or cannot be surgically removed without severe harm, and has progressed after prior treatment or has no satisfactory alternative therapy.

The NTRK gene fusion approval in solid tumors was granted under accelerated approval, meaning it is based on tumor response rates. Continued approval may depend on results from confirmatory clinical trials.
