Why this applies to Ibrutinib

Ibrutinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sweet Annie interacts with.

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.

Here's the deal with Imbruvica (ibrutinib) and Sweet Annie. Ibrutinib relies on a liver enzyme called CYP3A4 to be cleared from your body. Sweet Annie contains artemisinin, and lab studies pull in two different directions: one constituent may speed that enzyme up, while the whole extract may actually slow it down. So we honestly can't say for sure which way the scale tips in a real person.

Why does this matter? Ibrutinib isn't a prodrug, so if its breakdown slows, levels could climb and raise the risk of bruising, bleeding, irregular heartbeat, diarrhea, or infections. If its breakdown speeds up instead, the cancer medicine could become less effective. Since this is only test-tube evidence and ibrutinib is a serious drug, please clear Sweet Annie with your oncologist or pharmacist first.

What Is Ibrutinib Used For?

Brand name: Imbruvica

Imbruvica (ibrutinib) is approved for adult patients with chronic lymphocytic leukemia (CLL) / small lymphocytic lymphoma (SLL), including those with a specific genetic change called 17p deletion, and for adult patients with Waldenström's macroglobulinemia (WM), a rare type of blood cancer.

Imbruvica is also approved for chronic graft-versus-host disease (cGVHD) — a complication that can occur after a stem cell transplant — in adults and children aged 1 year and older whose condition did not respond to at least one prior treatment.
