Why this applies to Ivacaftor

Ivacaftor falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sweet Annie interacts with.

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.

Here's the situation with Kalydeco (ivacaftor) and Sweet Annie. Ivacaftor is mostly cleared from your body by a liver enzyme called CYP3A4, and Sweet Annie can tug on that same enzyme. The tricky part is that the lab data points in two directions: one piece of the herb (artemisinin) may speed the enzyme up, while the whole extract seems to slow it down. So we honestly can't say for sure which way it would push your ivacaftor levels.

If the enzyme sped up, ivacaftor could drop and work less well on your mucus and breathing. If it slowed down, levels could rise and you might notice more headache, dizziness, belly upset, or rash. This is only test-tube evidence, so it's not a sure thing. Please run it by your CF team or pharmacist before adding Sweet Annie.

What Is Ivacaftor Used For?

Brand name: Kalydeco

Kalydeco (ivacaftor) is used to treat cystic fibrosis (CF) in patients aged 1 month and older who have at least one mutation in the CFTR gene that responds to ivacaftor. A specific genetic test is used to confirm whether a patient's mutation qualifies.

There is a long list of CFTR mutations for which Kalydeco is approved — including G551D, R117H, F508C, S549N, and many others. If your or your child's mutation is unknown, an FDA-cleared CF mutation test should be done first to confirm eligibility.
