Perampanel falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sweet Annie interacts with.

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.

Here's the situation with Sweet Annie and your seizure medicine perampanel (Fycompa). Perampanel isn't a prodrug, meaning it works as-is, and your liver clears it using an enzyme called CYP3A4. Sweet Annie can tug on that enzyme in two directions. One of its compounds (artemisinin) may speed CYP3A4 up, which could clear perampanel faster and weaken its effect, raising the risk of breakthrough seizures. Yet the whole extract may also slow that enzyme, which could let perampanel build up and bring on more dizziness, drowsiness, trouble walking, or mood changes.

The honest part: this is based only on lab tests, not real people, so we truly don't know which way it tips in your body. Because seizure control matters so much, please check with your pharmacist or doctor before combining them.

What Is Perampanel Used For?

Brand name: Fycompa

Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.

Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
