Why this applies to Rifabutin

Rifabutin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sweet Annie interacts with.

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.

Your body clears rifabutin using a liver enzyme called CYP3A4, and Sweet Annie may bump into that same pathway. Here's the tricky part: in lab studies, the artemisinin in Sweet Annie actually sped up this enzyme, while the whole plant extract seemed to slow it down. So the direction isn't settled, and all of this comes from test-tube research, not real people.

If Sweet Annie speeds CYP3A4 up, rifabutin could clear faster and work less well, which matters when you're trying to prevent a serious infection like MAC. If it slows the enzyme, rifabutin levels could climb, raising the odds of side effects like nausea, a harmless orange tint to urine and tears, or low blood counts. Because we just don't know which way it goes here, please check with your pharmacist or doctor before combining them.

What Is Rifabutin Used For?

Brand name: Mycobutin

Rifabutin capsules are used to prevent disseminated MAC disease — a serious bacterial infection caused by Mycobacterium avium complex — in patients with advanced HIV infection.

It is not approved to treat active tuberculosis, and it should not be used alone against active TB. It is also not proven to prevent M. tuberculosis infections.
