Why this applies to Thiotepa

Thiotepa falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sweet Annie interacts with.

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP3A4, possibly increasing CYP3A4 activity by 1.9-fold. However, Sweet Annie extract seems to inhibit the activity of CYP3A4 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP3A4 substrates.

Here's the wrinkle with thiotepa: it's actually a prodrug. Your liver uses the CYP3A4 enzyme to convert thiotepa into TEPA, the form that does much of the cancer-fighting work and also drives toxicity. So anything that nudges CYP3A4 could shift the balance of active drug you end up with.

Sweet Annie's effect on this enzyme is genuinely murky. In lab dishes, the artemisinin in it sped CYP3A4 up, but the whole-plant extract slowed it down. We honestly don't know which wins in a real person, and this is only test-tube data, not human studies. Since thiotepa is a serious chemotherapy drug with a tight safety margin, please don't add Sweet Annie without first clearing it with your oncology team or pharmacist.

In vitro research shows that the Sweet Annie constituent artemisinin induces CYP2B6, possibly increasing CYP2B6 activity by 1.6-fold. However, Sweet Annie extract seems to inhibit the activity of CYP2B6 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP2B6 substrates.

Here's the wrinkle with this one: thiotepa is actually a prodrug. Your liver uses an enzyme called CYP2B6 to convert it into TEPA, the form that does much of the cancer-fighting work. So anything that changes that enzyme can shift how much active drug you end up with, which matters a lot for a chemotherapy medicine where dosing is carefully calculated.

Now, the honest truth is the evidence here is shaky. It comes only from lab dish studies, and they point in opposite directions: pure artemisinin seemed to speed the enzyme up, while whole Sweet Annie extract slowed it down. We simply don't know the real-world effect in people. Because thiotepa is serious chemo, please don't add Sweet Annie without first clearing it with your oncology team or pharmacist.

While the purified drug is well studied for malaria, the herb itself as a supplement is not proven for the wide range of conditions people use it for, and self-treating malaria with the raw herb is dangerous. About Thiotepa Injection

What Is Thiotepa Injection Used For?

Brand names: Thioplex, Tepadina

Tepadina (thiotepa) injection is used for several purposes. It treats adenocarcinoma (a type of cancer) of the breast or ovary. It is also used to control malignant effusions — abnormal fluid buildup in body cavities caused by cancer spreading to those areas. For bladder cancer, Tepadina treats superficial papillary carcinoma of the urinary bladder.

Tepadina is also used in pediatric patients with class 3 beta-thalassemia (a severe inherited blood disorder). In those patients, it is given as part of a preparative regimen — a conditioning treatment to reduce the risk of graft rejection before an allogeneic stem cell transplant (a transplant using donor stem cells), alongside the chemotherapy drugs busulfan and cyclophosphamide.
