Why this applies to Avacopan

Avacopan falls under HEPATOTOXIC DRUGS — the group Sweet Clover interacts with.

Theoretically, concomitant use of sweet clover with hepatoxic drugs might increase the risk of hepatotoxicity.
Orally, large amounts of sweet clover can cause transient liver injury in susceptible individuals, possibly due to coumarin content. In one case report, a patient with multiple sclerosis developed jaundiced palms and a slight elevation in serum alanine aminotransferase (ALT) after taking sweet clover containing coumarin 10 mg daily for 3 years. Two weeks after starting therapy with subcutaneous interferon (IFN)-beta 1b, aspartate transaminase (AST) and ALT levels increased to 7 and 17 times the upper limit of normal, respectively. Levels normalized after discontinuation of both products. Subsequent use of IFN-beta 1a alone, without sweet clover, resulted in only a very slight increase in AST.

Here's the thing to keep in mind with Avacopan (Tavneos): it's already a medication that can stress the liver. Your doctor likely checks your liver enzymes (those AST and ALT blood tests) before and during treatment for exactly that reason. Sweet clover contains coumarin, and in a few people, larger supplement-level amounts have been linked to mild, temporary liver irritation.

So the worry is simple addition. Putting a possible liver-stressor (sweet clover) on top of a drug that can also affect the liver might raise your chances of liver trouble, things like fatigue, nausea, belly pain on the right side, or yellowing of the skin or eyes. This is theoretical, based on how each one works, not proven for this exact pair. Still, I'd hold off and check with your pharmacist or doctor first.

What Is Avacopan Used For?

Brand name: Tavneos

Tavneos (avacopan) is used to treat severe active ANCA-associated vasculitis in adults — specifically the forms known as granulomatosis with polyangiitis (GPA) and microscopic polyangiitis (MPA). These are serious autoimmune conditions in which the immune system attacks small blood vessels, causing damage to the kidneys, lungs, and other organs.

Tavneos is used as an add-on treatment alongside standard therapies, including glucocorticoids (steroid medicines). It is not a replacement for steroids — it works together with them.
