Why this applies to Elotuzumab

Elotuzumab falls under HEPATOTOXIC DRUGS — the group Sweet Clover interacts with.

Theoretically, concomitant use of sweet clover with hepatoxic drugs might increase the risk of hepatotoxicity.
Orally, large amounts of sweet clover can cause transient liver injury in susceptible individuals, possibly due to coumarin content. In one case report, a patient with multiple sclerosis developed jaundiced palms and a slight elevation in serum alanine aminotransferase (ALT) after taking sweet clover containing coumarin 10 mg daily for 3 years. Two weeks after starting therapy with subcutaneous interferon (IFN)-beta 1b, aspartate transaminase (AST) and ALT levels increased to 7 and 17 times the upper limit of normal, respectively. Levels normalized after discontinuation of both products. Subsequent use of IFN-beta 1a alone, without sweet clover, resulted in only a very slight increase in AST.

Here's the simple version: sweet clover contains coumarin, which in large or concentrated supplement doses can occasionally stress the liver in sensitive people. Elotuzumab (Empliciti) isn't broken down by the liver the way pills are, since it's an antibody given by infusion. So this isn't a true metabolism clash. The concern is more about stacking two things that can each tax the liver, which could make liver enzyme bumps more likely.

Keep in mind this is theoretical, based on how sweet clover behaves, not on solid studies with elotuzumab. Also remember elotuzumab is usually paired with lenalidomide or pomalidomide plus dexamethasone, which adds more reasons to protect your liver. Since you're being treated for myeloma, please check with your oncology team or pharmacist before adding sweet clover.

About Elotuzumab Injection

What Is Elotuzumab Injection Used For?

Brand name: Empliciti

Empliciti is used to treat multiple myeloma (a cancer of plasma cells in the bone marrow) in adults who have already tried other treatments. It is always given as part of a combination regimen — never alone.

Empliciti combined with lenalidomide and dexamethasone is used in adults who have received one to three prior therapies. Empliciti combined with pomalidomide and dexamethasone is used in adults who have received at least two prior therapies, including lenalidomide and a proteasome inhibitor.
