HEPATOTOXIC DRUGS
Why this applies to Lesinurad, Allopurinol
Lesinurad, Allopurinol falls under HEPATOTOXIC DRUGS — the group Sweet Clover interacts with.
Orally, large amounts of sweet clover can cause transient liver injury in susceptible individuals, possibly due to coumarin content. In one case report, a patient with multiple sclerosis developed jaundiced palms and a slight elevation in serum alanine aminotransferase (ALT) after taking sweet clover containing coumarin 10 mg daily for 3 years. Two weeks after starting therapy with subcutaneous interferon (IFN)-beta 1b, aspartate transaminase (AST) and ALT levels increased to 7 and 17 times the upper limit of normal, respectively. Levels normalized after discontinuation of both products. Subsequent use of IFN-beta 1a alone, without sweet clover, resulted in only a very slight increase in AST.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the concern with Duzallo (which combines lesinurad and allopurinol) and sweet clover. Both of these gout medicines can occasionally stress the liver on their own. Allopurinol in particular is known to rarely cause liver enzyme changes, and lesinurad can affect the liver and kidneys too. Sweet clover, because of its coumarin content, has also been linked to liver irritation in some people, especially in concentrated supplement doses or if the plant has spoiled.
So the worry is simply additive: stacking two things that can each bother the liver. This is theoretical, based on how they work rather than strong studies, so don't panic. But before combining them, please check with your pharmacist or doctor, who may want to watch your liver tests.

From our pharmacist’s overview on the Lesinurad, Allopurinol page
“Kidney-harming herbs are the sharpest hazard on this page. Siberian cocklebur and asarabacca (often contaminated with aristolochic acid) both carry major ratings, and that matters because lesinurad, half of the Duzallo combination for gout, comes with its own kidney injury warning.”

About Sweet Clover
What is Sweet Clover?
- Varicose veins and poor circulation
- Leg swelling (edema)
- Hemorrhoids
- Bruising and minor inflammation
- Respiratory complaints
Sweet clover is a traditional European herb most often promoted for vein problems and leg swelling, but high-quality human evidence is limited. It contains coumarin, which can become dangerous if the plant spoils, so quality matters and you should talk to a pharmacist before using it.
