HEPATOTOXIC DRUGS
Why this applies to Mirikizumab-mrkz
Mirikizumab-mrkz falls under HEPATOTOXIC DRUGS — the group Sweet Clover interacts with.
Orally, large amounts of sweet clover can cause transient liver injury in susceptible individuals, possibly due to coumarin content. In one case report, a patient with multiple sclerosis developed jaundiced palms and a slight elevation in serum alanine aminotransferase (ALT) after taking sweet clover containing coumarin 10 mg daily for 3 years. Two weeks after starting therapy with subcutaneous interferon (IFN)-beta 1b, aspartate transaminase (AST) and ALT levels increased to 7 and 17 times the upper limit of normal, respectively. Levels normalized after discontinuation of both products. Subsequent use of IFN-beta 1a alone, without sweet clover, resulted in only a very slight increase in AST.

Here's the thinking behind pairing sweet clover with Omvoh (mirikizumab). Omvoh is an injectable antibody used for ulcerative colitis, and like some biologics it can occasionally stir up the liver, showing up as elevated liver enzymes on blood work. Sweet clover contains coumarin, which in large or long-term amounts has been linked to liver irritation on its own. So the worry is simple addition: two things that can each nudge the liver, used together, might raise the odds of liver stress a bit more.
To be honest with you, this is a theoretical concern based on how these ingredients behave, not proof it will happen. Still, since your care team likely checks liver labs while you're on Omvoh, mention any sweet clover product to your pharmacist or doctor first, and watch for fatigue, yellowing skin or eyes, or dark urine.

What Is Mirikizumab-Mrkz Used For?
Brand name: Omvoh
Omvoh (mirikizumab-mrkz) is used to treat moderately to severely active ulcerative colitis in adults — a condition where the large intestine becomes chronically inflamed and ulcerated. It is also used to treat moderately to severely active Crohn's disease in adults, another form of inflammatory bowel disease that can affect any part of the digestive tract.
Both uses are specific to Omvoh administered by injection — first as intravenous (into the vein) infusions to start treatment, then as subcutaneous (under the skin) injections for ongoing maintenance.
