Benzhydrocodone, Acetaminophen falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Thunder God Vine interacts with.
In vitro research shows that triptolide, a constituent of thunder god vine, inhibits CYP1A2 enzymes in human microsomes. This interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Apadaz (benzhydrocodone plus acetaminophen) and Thunder God Vine. In lab dish studies, a compound in this herb called triptolide seems to slow down a liver enzyme known as CYP1A2. If that enzyme is dialed back, drugs it helps clear could linger a bit longer in your system. In theory, that might nudge up opioid-type effects like drowsiness, dizziness, constipation, nausea, or slowed breathing.
That said, I want to be straight with you: this is theoretical only, based on test-tube work, and it has not actually been reported in people. Thunder God Vine also carries its own safety concerns and really should only be used under a doctor's watch. Please run this combo by your pharmacist or doctor before mixing the two.

Here's the thing about Apadaz: the benzhydrocodone part is actually a prodrug, meaning your body has to convert it into hydrocodone before it does much for pain. There's a wrinkle, though, because once it becomes hydrocodone, your liver's CYP3A4 enzyme helps break it down and clear it out. Thunder God Vine, at least in lab dish studies, may block that CYP3A4 enzyme.
If that blocking happened in real life, your opioid could hang around longer and build up, leading to stronger effects like heavy drowsiness, constipation, nausea, or slowed, shallow breathing. I want to be honest though: this is theoretical, based only on test-tube data, and has never been reported in people. Still, since opioids are nothing to gamble with, please check with your pharmacist or doctor before combining them.

Apadaz is indicated for the short-term management of acute pain — up to 14 days — that is severe enough to require an opioid and for which non-opioid treatments have not worked or are not expected to work. It combines benzhydrocodone (a prodrug that converts to the opioid hydrocodone) with acetaminophen.
Because of serious risks including addiction, abuse, and misuse, Apadaz is reserved for patients who have not tolerated or have not gotten adequate relief from alternative pain treatments. It should not be used for an extended period of time unless the pain remains severe enough to require an opioid and alternatives remain inadequate.
