Fosaprepitant falls under CYTOCHROME P450 3A4 (CYP3A4) INHIBITORS — the group Thunder God Vine interacts with.

Theoretically, CYP3A4 inhibitors might increase the levels and adverse effects of thunder god vine constituents.
Inhibitors of CYP3A4 can increase the concentration of triptolide, a thunder god vine constituent, and its derivative (5R)-5-hydroxytriptolide. In vitro research shows that triptolide and (5R)-5-hydroxytriptolide are about 94% and 64% metabolized by CYP3A4, respectively.

Here's the situation with these two. Fosaprepitant (Emend) is the IV form given to prevent nausea and vomiting from chemo, and it works partly by slowing down a liver enzyme called CYP3A4. Thunder God Vine's active compounds, especially triptolide, are largely broken down by that same enzyme. So in theory, the Emend could cause the herb to build up in your system, which may raise the risk of Thunder God Vine's side effects (it can be quite toxic when levels run high).

I want to be honest about the strength of this: it's based on lab studies and how these substances behave on paper, not on real patient reports. Still, since Thunder God Vine can be harmful even on its own, please loop in your oncology team or pharmacist before combining them.

In vitro research shows that triptolide, a constituent of thunder god vine, inhibits CYP3A4 enzymes in human microsomes. This interaction has not been reported in humans.

Here's the quick picture with these two. Fosaprepitant (Emend) is the IV form that your body quickly turns into aprepitant, the medicine that helps prevent nausea and vomiting from chemotherapy. Aprepitant is partly handled by a liver enzyme called CYP3A4. Thunder God Vine, based only on lab dish studies of its compound triptolide, may slow that same enzyme down a little.

In theory, that could let aprepitant build up slightly and lead to more of its usual effects, like tiredness, hiccups, or mild stomach upset. I want to be honest though: this has never actually been reported in real people, so it stays a paper concern. Since Thunder God Vine itself can be toxic and Emend is given during chemo, please loop in your oncology team before combining them.
