Bedaquiline falls under Oral Drugs — the group Thyroid Extract interacts with.

Food and medications can interfere with thyroid hormone absorption. Some foods, particularly those high in fiber or containing certain minerals like calcium or iron, can bind to thyroid hormones and reduce their absorption in the digestive tract. Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.

Here the concern runs the other way from what you might expect. It is not that bedaquiline changes your thyroid extract, but that taking the two together at the same time can blunt how well your body absorbs the thyroid hormone. Bedaquiline (Sirturo) is taken with food on purpose, since food roughly doubles how much you absorb. Thyroid hormones, on the other hand, want an empty stomach. So if you swallow them together, you may soak up less of the thyroid hormone.

This is a minor, mostly timing-based issue. A simple fix is spacing them apart (thyroid hormone first, at least 30 to 60 minutes before eating and before your bedaquiline dose). Since you are treating serious TB, please let your doctor or pharmacist set the schedule rather than adjusting anything yourself.

About Bedaquiline

What Is Bedaquiline Used For?

Brand name: Sirturo

Sirturo (bedaquiline) is used as part of a combination treatment for pulmonary TB — TB that affects the lungs — caused by bacteria resistant to at least both rifampin and isoniazid. It is approved for adults and for children 2 years and older who weigh at least 8 kg (about 17.6 lbs).

Sirturo is not used for latent TB (TB infection with no active symptoms), TB outside the lungs, drug-sensitive TB, or infections caused by non-tuberculosis mycobacteria. It must always be used alongside other TB medicines — never alone.
