Bedaquiline falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Cannabidiol may increase levels of drugs that are metabolized by CYP3A4.
In vitro and animal research shows that cannabidiol inhibits CYP3A4. In human studies and case reports, cannabidiol has been associated with an increase in plasma levels of the CYP3A4 substrates zonisamide, tacrolimus, everolimus, citalopram, midazolam, and methadone.

Here's the gist: your body uses an enzyme called CYP3A4 to break down bedaquiline, and CBD can slow that enzyme down. When the enzyme works slower, bedaquiline can build up higher than expected in your bloodstream over time.

Why this matters for bedaquiline specifically: it can affect your heart's rhythm (a change called QT prolongation that shows up on an EKG), and higher levels may make that more likely, sometimes alongside liver irritation. Bedaquiline is also part of a carefully balanced TB regimen, so you don't want surprise changes. The signal that CBD blocks CYP3A4 is well supported, though it hasn't been tested directly with this exact drug. Please don't start or stop anything on your own. Talk with your TB doctor or pharmacist first, since they may want to check your EKG.

What Is Bedaquiline Used For?

Brand name: Sirturo

Sirturo (bedaquiline) is used as part of a combination treatment for pulmonary TB — TB that affects the lungs — caused by bacteria resistant to at least both rifampin and isoniazid. It is approved for adults and for children 2 years and older who weigh at least 8 kg (about 17.6 lbs).

Sirturo is not used for latent TB (TB infection with no active symptoms), TB outside the lungs, drug-sensitive TB, or infections caused by non-tuberculosis mycobacteria. It must always be used alongside other TB medicines — never alone.
